Preparing for Your Blood Test
To schedule a blood test, patients are required to present a laboratory request form that has been duly signed and stamped by a licensed physician (AGB code). Additionally, they must bring their Arizona Vehicle Registration (AZV) card. In the event that a patient does not possess an AZV card, valid identification must be provided.

The laboratory request form must be meticulously completed with the following details:
a. Patient’s full name
b. Date of birth
c. Contact information (including address and phone number)
d. AZV number
e. The name, stamp, and signature of the physician or specialist who has authorized the test

Preparing for Laboratory Tests
To schedule a laboratory test, you must submit a laboratory request form signed by a licensed physician. This typically involves blood and/or urine tests, although you may also need to provide additional body samples such as stool, semen, or other specimens for analysis. Most tests can be performed at any ML or IMSan location without prior appointment.
Please carefully review the laboratory request form provided by your physician, as it contains crucial instructions. These may include specific requirements, such as whether fasting is necessary before the test.
Fasting entails refraining from consuming food or beverages after 10:00 PM on the day preceding the test. Water or plain tea without additives are permitted. If you are taking medication, you may continue taking it as prescribed, unless your physician advises otherwise.

Upon presenting for specimen collection, your consent for the sample to be taken and analyzed is understood. The responsibility for obtaining informed consent for the tests lies with the medical personnel who order them. This consent should encompass all tests being conducted, the potential consequences of the results, and the sharing of clinical and personal information with relevant healthcare providers.
This process conforms to the ISO 15189:2022 standard.
Certain tests may not be available at every ML or IMSan location or at all times. The specific location for your test will be indicated on your laboratory form or communicated to you by your physician. If you require materials for your test, such as a container for urine collection, these can be obtained at any location.

Informed Consent for the Collection of Body Samples for Diagnostic Testing
Any body samples taken for laboratory diagnostic purposes must be accompanied by informed consent from the patient. This ensures that you, as the patient, are fully cognizant of the procedure and the rationale behind the test.
For instance, when a blood sample is drawn, the healthcare professional administering the procedure presumes that you have received comprehensive information from your physician and have consented to the blood test being conducted.
