Prior to a diagnosis or evaluation of treatment efficacy, your physician, specialist, or midwife may necessitate laboratory tests on samples such as blood, urine, or stool. These tests are conducted in the Clinical Chemistry and Hematology laboratory.
The laboratory utilizes two types of request forms
- The red form is specifically designed for blood tests.
- The yellow form is utilized for urine tests.
Please ensure that the request form is meticulously completed before submission. The patient’s details should be prominently displayed in the top left corner of the form.

The necessary materials and forms for sample collection can be procured through the laboratory’s warehouse, via email, or by telephone at 5861600.
Test Result Distribution
Test results are disseminated to the requesting physician, medical specialist, and the designated ward of the patient.
Furthermore, test results are accessible via the IMSan hospital and laboratory result systems across various departments.
General practitioners will receive test results electronically through Promedico.
